Everybody knows some brand failures: global trademarks that have some funny or negative kind of connotation in a country or in a different language. Ikea has been confronted with the most brand failures with trademarks as Jerker, Fukta and Fartfull. An other is Emu Air (Emu is a bird that cannot fly).
Not very common are design failures. But Markmatters.com has one: the teapot of JcPenny. The teapot on the billboard just shows a teapot, but if you pass by real quickly you might see Adolf Hitler, not directly the kind of person you wish to relate to you new product!
